WELLNESS PLAN INFORMATION

 

Cordova Animal Medical Center is committed to offering quality affordable preventative care.    In order to enable our clients to  be able to give their pet the best,  we have designed several wellness plans to fit their needs.   We offer four levels of Wellness Plans starting from a bare bones Basic Plan to an all inclusive Senior Plan. 

 

Clients can choose their level of Wellness Plan and choose to pay by either an upfront yearly fee or a low monthly payment.    All plans include discounts on examinations as an additional bonus.    Please contact the office for details.   Please don't let cost prevent you from getting your pet the care they deserve. 

 

BASIC WELLNESS PLAN:   All pets $200 advance   or  $21 per month payments

 Includes the following services for one year:

                                *Economy Annual (Exam,  Rabies, and DHLPP-dogs  or FVRCP-cats)  Economy annuals do not include heartworm testing or fecal examination.   Heartworm is required to purchase heartworm preventative.

                                *Economy Dental (Anesthesia with IV catheter and fluids, scaling, polishing, fluoride application).  Economy dentals do not include oral examinations, dental x-rays, antibiotic injections, probing or charting.    The economy dental does not include pre anesthetic lab testing (required for pets 7 and over)  or pre anesthetic ECGs (required for pets with murmurs or history of heart disease).  

                                *10% off all examinations (regular, brief, recheck, and emergency)

 

BETTER WELLNESS PLAN:  Pets under 40 lbs  $346 advance  or $34 per month payments

                    Pets over    40 lbs $396 advance  or $38 per month payments

Includes the following services for one year:

                                *Regular Annual (Exam,  All vaccines needed- Rabies, DHLPP, Bordatella for dogs,  Rabies,  FVRCP, Feline Leukemia for cats,  antigen heartworm test- dogs,  intestinal parasite screen or fecal) 

                                *Regular Dental (Anesthesia with IV catheter and fluids, scaling, polishing, fluoride applications, full mouth dental x-rays, oral examination, probing, and charting).  The Regular Dental does not include pre anesthetic lab testing (required for pets 7 and over)  or pre anesthetic ECGs (required for pets with murmurs or history of heart disease).

                                *15% off all examinations (regular, brief, recheck, and emergency)

 

BEST WELLNESS PLAN:   Pets under 40 lbs  $408 advance  or $40 per month payments

                  Pets over 40 lbs $458 advance  or $44 per month payments

 Includes the following services for one year:

                                *Regular Annual (Exam,  All vaccines needed- Rabies, DHLPP, Bordatella for dogs,  Rabies,  FVRCP, Feline Leukemia for cats,  antigen heartworm test- dogs,  intestinal parasite screen or fecal) 

                                *Junior Wellness Labwork (CBC, Chemistry)

                                *Regular Dental (Anesthesia with IV catheter and fluids, scaling, polishing, fluoride applications, full mouth dental x-rays, oral examination, probing, and charting).  The Regular Dental does not include pre anesthetic lab testing (required for pets 7 and over)  or pre anesthetic ECGs (required for pets with murmurs or history of heart disease).    The junior wellness lab work can serve as pre anesthetic  lab testing if performed within one month of the dental services.

                                *20 % off all examinations (regular, brief, recheck, and emergency)

 

SENIOR WELLNESS PLAN:  Pets under 40 lbs  $732 advance  or $67 per month payments

                   Pets over    40 lbs $782 advance  or $71per month payments

Includes the following services for one year:

                                *Regular Annual (Exam,  All vaccines needed- Rabies, DHLPP, Bordatella for dogs,  Rabies,  FVRCP, Feline Leukemia for cats,  antigen heartworm test- dogs,  intestinal parasite screen or fecal) 

                                *Senior Wellness Labwork (CBC, Chemistry, Urinalysis, Thyroid- T4, Microalbumin)

                                *ECG (pre anesthetic screening)  

                                *Chest X-rays  3 view

                                *Regular Dental (Anesthesia with IV catheter and fluids, scaling, polishing, fluoride applications, full mouth dental x-rays, oral examination, probing, and charting).  The Regular Dental does not include pre anesthetic lab testing (required for pets 7 and over)  or pre anesthetic ECGs (required for pets with murmurs or history of heart disease).    The senior wellness lab work can serve as pre anesthetic  lab testing if performed within one month of the dental services.

                                *20 % off all examinations (regular, brief, recheck, and emergency)

 

 

*WHAT IS NOT INCLUDED

                - The Dentals do not include pre anesthetic lab testing (required for pets 7 and over)  or pre anesthetic ECGs (required for pets with murmurs or history of heart disease).   

-Dentals do  NOT include treatment of any  problems are detected during the procedure(does not include extractions, periodontal treatment, restoration, or associated treatments)

- Wellness plans do not include laboratory testing,  professional services or procedures, surgical procedures,  grooming services,  county license fees,  drugs or products.    Only the procedures listed are included in the wellness plan.    Additional diagnostic testing or treatments of conditions diagnosed by wellness plan services are not included.  

-Economy dental clients that have pets 7 and up will need to plan to pay for pre anesthetic lab testing or ecg charges (if murmur or heart disease history)-  they can have the option to add the amount of these services to the prepaid amount or payment amount to spread it out through the year.   Will require an addendum to the agreement.

 

*IF PAY BY ADVANCE PAYMENT- CAN UTILIZE SERVICES IMMEDIATELY

 

*IF PAY BY PAYMENTS   START OF USAGE IS DETERMINED BY THE FOLLOWING:

             -  CAN UTILIZE ANNUALS AFTER 1ST MONTHLY PAYMENT IS MADE

                                             -   CAN UTILIZE LAB AND DENTALS AFTER 2ND MONTHLY PAYMENT IS MADE

                                             -CAN UTILIZE SENIOR ECG AND X-RAY AFTER 3RD MONTHLY PAYMENT IS MADE

        **IF ALREADY A CLIENT IN GOOD STANDING, THIS CAN BE WAIVED BY DOC
